چکیده

METHODS Participants were interviewed for 30–80 minutes with open ended questions that were modified using the process of theoretical sensitivity and theoretical sampling. Data were also collected from conversations with staff and observations of on-duty staff (3–8 h). Interviews were tape recorded, transcribed, and thematically coded. 83 conceptual categories and subcategories were generated and integrated by the process of constant comparison.

Performance of Early Warning Scoring Systems to Detect Patient Deterioration in the Emergency Department

Acute hospital wards in the UK are required to use Early Warning Scoring (EWS) systems to monitor patients’ vital-signs. These are often paper-based, and involve the use of heuristics to score the vital signs which are measured every 2-4 hours by nursing staff. If these scores exceed pre-defined thresholds, the patient is deemed to be at risk of deterioration. In this paper we compare the perfo...

Early warning scores: a sign of deterioration in patients and systems.

The early warning score system is a decision-making tool that has a simple design, yet its implementation in healthcare organisations is proving complex. This article reports the results of a survey that evaluated nurses' experiences of using the National Early Warning Score (NEWS) in an acute hospital in Ireland. Staff reported that the NEWS was easy to use, did not increase workload and enhan...

Continuous Monitoring of Vital Signs Using Wearable Devices on the General Ward: Pilot Study

BACKGROUND Measurement of vital signs in hospitalized patients is necessary to assess the clinical situation of the patient. Early warning scores (EWS), such as the modified early warning score (MEWS), are generally calculated 3 times a day, but these may not capture early deterioration. A delay in diagnosing deterioration is associated with increased mortality. Continuous monitoring with weara...

Machine Learning for the Deterioration of Patients on Hospital Wards

Early Warning Score Systems assign scores to a patient’s vital signs to detect adverse events in order to secure timely medical response. However, most systems do not account for variations in vital signs due to aging changes or sex differences, which affects the accuracy of current systems for different deteriorating patient sub-groups. We aimed to develop an Ageand Sex-Based Early Warning Sco...

Barriers and facilitating factors related to use of early warning score among acute care nurses: a qualitative study

BACKGROUND The early warning score (EWS) was developed to identify deteriorating patients early. It is a track-and-trigger system based on vital signs designed to direct appropriate clinical responses based on the seriousness and nature of the underlying condition. Despite its wide dissemination, serious adverse events still occur, often due to failure among staff on general wards to follow the...
